Self-pay patients and those with PPO insurance — we are excited to offer you a convenient and streamlined process to schedule your new patient appointment.

To make this process more convenient for you, we’ve introduced a new way to schedule your appointment. We now offer the option for you to upload your medical information, including your most recent primary care provider notes, before coming coming to the office.

How It Works

Submit Your Information

Click the "New Patient" button below to access the secure form. Fill out your details and upload your medical documents, including any recent primary care provider notes. This step ensures we have a complete view of your health history. If you are using a mobile device, please be sure to do so at a time you can go uninterrupted. Leaving the form at any time will result in you having to start over.

Review and Contact

After you submit your information, our team will review it to ensure everything is complete. We want to make sure we have all the necessary information to schedule your appointment without delays.

We are located in at 850 Hickory Street, Suite 103, Abilene, Texas 79601.
When heading west on Ambler take a left on Hickory. We are on the right, on the corner of N. 18th & Hickory. Inside the building we are on the first floor, sixth door on the left.
We take all insurance, Hendrick however does not accept Ambetter if you are needing any services rendered from them separately.
If you have a PPO or are self pay you do not need a referral. If you have Tricare Prime, Blue Cross Blue Shield HMO, Health Select, and Triwest VA, do require referral authorization.

At every appointment we need your an insurance card, valid ID, and current list of medications. 

Self pay client’s initial new patient visit is $200.00, follow up appointments are $100.00 each visit.
